I, the parent/guardian/caregiver listed above, on behalf of myself and my child, hereby agree to participate in the Lewis County Autism Coalition Neurodiverse Connections Teen Group (hereinafter referred to as "the Program") organized by the Lewis County Autism Coalition. I understand that the physical location of the teen groups will be held at community designated locations of which LCAC has notified parents/guardians/caregivers.
In consideration of my child's participation in the Program, I hereby acknowledge and agree to the following:
1. Assumption of Risk: I understand that participation in the Program involves inherent risks, including but not limited to physical injury, illness, or other potential harm. I voluntarily assume all risks associated with my child's participation in the Program. I understand that I am required to either stay on the premises or close enough by (i.e., parking lot) to be reached immediately in the event of necessity.
2. Release of Liability: I hereby release, waive, and discharge the Lewis County Autism Coalition, its officers, directors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including but not limited to bodily injury or property damage, that may be sustained by my child or me during the Program.
3. Indemnification: I agree to indemnify and hold harmless the Lewis County Autism Coalition, its officers, directors, employees, volunteers, and agents, as well as any community partners who share their space or hold events involved with the Neurodiverse teen group at their space, from any and all liability, claims, demands, actions, or causes of action arising out of or related to my child's participation in the Program.
4. Medical Treatment: I authorize the Lewis County Autism Coalition and its representatives to obtain medical treatment for my child in the event of any injury or illness that may occur during the Program, and I agree to be financially responsible for any costs associated with such medical treatment.
5. Governing Law: This waiver and release of liability shall be governed by and construed in accordance with the laws of the State of Washington.
